Required inputs

  • Hypertension (SBP > 160)
  • Abnormal renal function (dialysis, transplant, SCr ≥ 2.26 mg/dL / 200 µmol/L)
  • Abnormal liver function (cirrhosis, bilirubin >2×, AST/ALT/ALP >3×)
  • Prior stroke
  • Prior major bleeding / predisposition
  • Labile INR (TTR < 60%)
  • Age > 65
  • Concomitant antiplatelet / NSAID
  • Alcohol ≥ 8 drinks / week
